In order
to expedite the processing of your RMA number, please fill
out this form completely and click the "Submit"
button below. Once your RMA request is processed, we will
send you an Return Materials Authorization (RMA) number via
e-mail, along with shipping instructions for the return of
your Fantom Drives Product within 24 hours. After receiving
your RMA number, please fax us a copy of your "Original
Purchase Invoice" or "Packing
List". The fax number will be listed on the RMA
you receive. .
You
may also print and fax this form by downloading our printable
version (PDF, requires Adobe Acrobat Reader) but processing
may be slightly delayed.
Please
note that the RMA number has a validity of 15 days upon issuance.
We must receive your product within 15 days upon issuance
of the RMA number,otherwise, the RMA number will be void and
you'll have to apply for a new one.
